Patient Information
Billing & Insurances We Participate With

Here is some information concerning our payment policies and a list of insurance plans that we participate with.

If you plan has a co-pay for office visits, this must be paid at the time of your visit. It will be collected at the front desk when you arrive for your appointment. In addition, if you plan only pays a percentage of our charges and this is a plan that we do not participate with, you will be billed for the difference.

If you have an insurance plan that we do not participate with, payment is expected at the time of the visit. We will submit the bill to your insurance as a courtesy to you. It is still your responsibility to follow up with the insurance company and make sure that the charges are paid, either by the insurance company or yourself.

The starred (*) insurances have plans that require referrals or prior authorization that must be obtained from your primary care physician PRIOR to your being seen in our office. Please check with your primary care physician to see if a referral is required for your visit to our office. If you do not have a referral for a particular visit, you will be asked to either reschedule your appointment or pay for the service, in full, at the time of the service.

We have agreed with the above insurance plans that we will accept the fee they allow for any claims that are submitted, however, the patient may still be responsible for any deductibles, amounts applied to co-insurance and co-pay.

If you are a member of a plan with which we do not participate we will submit your bill to that insurance plan, but you will be responsible for reimbursing our office for services provided at the time the service is rendered. Please be aware that each insurance company has their own fee schedule and may or may not pay the full amount that is billed. Insurance companies often state on their explanation of benefits that "the amount billed is not within the usual and customary fee". However, the balance will be the patients' responsibility. We will also make an attempt to verify your insurance before your surgery is performed, and will contact you regarding any amount that will not be covered by your insurance company.
